Download; Adobe PDF Or Microsoft Word (.docx)
Step 1 – Save The Motorcycle Maintenance Work Order Available On This Page
The “Adobe PDF” and “Microsoft Word (.docx)” links in this area of the page along with the two buttons underneath the sample image (“PDF” and “Word” respectively) will initiate a direct download of the Motorcycle Maintenance Work Order in the file version you select.
Step 2 – Deliver An Introduction Of The Motorcycle Maintenance And Repair Business
The Motorcycle Maintenance Shop or Mechanic being commissioned must be identified at the beginning of the work request. The “Company Name” text currently visible in the first text box presented should be cleared or deleted and substituted with the legal Motorcycle Maintenance Shop or Freelance Mechanic’s legal Business Name. After naming the Business being commissioned, turn your attention to the first blank line (labeled “Name”). Here, your entry must identify the Office Employee or Freelance Mechanic dealing with the Motorcycle Maintenance Customer and this paperwork.
The mailing address where the Motorcycle Maintenance Shop or the Freelance Mechanic prefers official correspondence and payments to be sent is also mandatory for this area of the work order. Three lines are available for your use in this report. Distribute the information making up this address on the “Street Address,” the “City, State, Country,” and “Zip Code” lines.
The final contact material required for the Motorcycle Maintenance Shop or Freelance Mechanic’s introduction is this Party’s “Phone” number(s) and “E-Mail” address. Notice that two clearly labeled lines have been put on display in this area to accept these entries.
Step 3 – Furnish The Request For Motorcycle Maintenance With Reference Material
The information that will be needed to store this request properly in the Motorcycle Maintenance Shop’s files (and the Customer’s) has an area of presentation reserved through the blank line labeled “Work Order #.” Deliver a report on this filing number to this line. It is customary to attach a document “Date” to filing material such as the work order number that you have supplied. The next available line in the shaded bar on this page will accept this “Date” for display.
Step 4 – Distribute The Motorcycle Maintenance Customer’s Billing Information
The Motorcycle Maintenance Customer will be the topic of focus in the “Client Information” section. Use the initial three lines of this section to dispense the “Name,” “E-Mail” address, and “Phone” number of the Motorcycle Maintenance Customer. Continue to the next segment of the Client Information section then deliver a production of the “Street Address,” “City, State, Country,” and “ZIP Code” where the Motorcycle Maintenance Customer expects his or her invoices and bills to arrive on the three lines bearing the appropriate labels.
Step 5 – Solidify When The Motorcycle Should Be Worked On
The area titled “Order” will require the dates when the motorcycle maintenance will be performed. The “Expected Start Date” is the first calendar date that should be recorded here. Find the first line in this area then enter this date accordingly. Proceed to the next labeled line (“Expected End Date”) then document the month, day, and year when the motorcycle maintenance will be completed.
Step 6 – Account For The Billable Motorcycle Parts Or Supplies
The request being made must now be solidified to a certain extent. First, submit a production of the motorcycle parts or motorcycle supplies (i.e. belts, oil, etc.) that should be billed to the Maintenance Customer in the “Description” column of the “Material” table. The second column will also account for the billable motorcycle parts and maintenance materials needed for this job by requesting that the number or “Quantity” that must be used and paid for be recorded on the same row in the adjacent column.
Continue to the third column then dispense the cost of the motorcycle part or supply that is documented in the first column. This “Unit Cost” must be the dollar amount expected for one product, part, piece, or unit of supply.
The “Amount” that will be owed for these parts will also need to be listed on a row by row basis. Thus, continue across each row to the final column, multiplying the reported “Quantity” by the “unit Cost” so that the result can be displayed as the “Amount” to be paid.
When you have performed the above tasks for every motorcycle part or maintenance supply that will be ordered and billed for the job, add every calculated dollar “Amount” in the final column to the “Total Material” field.
Step 7 – Display The Motorcycle Maintenance Service Time
The “Service” table displayed next will also need some material presented. The “Description” of the work shifts the motorcycle maintenance job will require for successful completion will be the first required production for this table. Therefore, furnish a production dates and work times of each day of work that will be needed for the job in the first column. Produce the number of “Hours” that will be billed for every recorded motorcycle maintenance work shift in the second column (on the same row).
Next, submit documentation of the “Hourly Rate” applied to the concerned motorcycle maintenance shifts in the third column.
The last column, or “Amount,” will allow a record of the payment required for each shift once you reach this figure by multiplying a shift’s “Hours” and “Hourly Rate.”
Add every motorcycle maintenance shift “Amount” to be charged to the “Total Service” field displayed at the bottom of the “Amount” column.
Step 8 – Submit The Totals For the Motorcycle Maintenance Job
A common courtesy afforded to Maintenance Customer is a production of the “Subtotal” the Motorcycle Maintenance Shop or Freelance Mechanic expects for the “Total Material” and “Total Service.” Thus, locate the “Subtotal” field introducing the next table. The “Tax” field appearing next seeks a submission of the determined sales or service taxes that may need to be collected with the motorcycle maintenance “Subtotal.” Apply the relevant “Tax” codes to the “Subtotal” then distribute the result to the appropriately labeled field.
The motorcycle maintenance job’s “Subtotal” and “Tax” must now be added to one another. This sum will be the payment expected for the motorcycle’s maintenance and/or repairs. Be advised, this is considered an estimate as opposed to a solid invoice or bill.
Step 9 – Identify This Work Order’s Processor
The individual processing this paperwork on behalf of the Motorcycle Maintenance Shop or Freelance Mechanic should be identified as the Submitter of the information presented. Place the name or names of this request’s Preparer(s) on the line labeled “Work Order Completed By…”
Step 10 – Give Further Discussion Regarding The Motorcycle Or Job As Needed
This requisition for motorcycle maintenance may require additional comments or either Party may wish to include additional or helpful comments. If so, then use the “Comments Or Suggestions” lines to furnish this material accordingly.
Step 11 – Obtain The Motorcycle Maintenance Customer’s Authorizing Signature
The “Client Signature” line at the end of this request is placed to receive proof of the Motorcycle Maintenance Customer’s approval both for the information above as well as the payment terms that will accompany the maintenance work requested.

Step 2 – Directly Address Motorcycle Mechanic Or Maintenance Center
The logo used by the Motorcycle Mechanic or Maintenance Shop should be placed on display in cell A1 in replacement of the image and placeholding label (“Add Your Company Logo Here”) it currently displays. The legal “Company Name” of the Motorcycle Mechanic or Maintenance Shop receiving this request is expected in cell A2 while the full “Name” of the individual Employee or Contractor reviewing the information that will set must be recorded in cell A3.
Now continue to the cell A4 and cell A5 where the first line and second line (respectively) of the Motorcycle Mechanic or Maintenance Center’s business address must be put on display.
Submit the “Zip Code” found in the Motorcycle Mechanic or Maintenance Shop’s business address as the content of cell A6.
The next two fields, cell A7 and A8, are reserved to present the “E-Mail” address and “Phone” number currently maintained by the Motorcycle Mechanic or Maintenance Shop.
Step 3 – Submit Definitions Needed To Catalog This Request Properly
Directly across the Motorcycle Mechanic (or Maintenance Shop) information, two more items are requested. First, supply cell F5 with the “Work Order” number that will be used to store the information this maintenance request presents to both Parties. Remember since the “Work Order” number is a filing tool, it should be kept unique in one’s records. The next field, “Date,” is filled in with your desktop’s current calendar “Date.” It will be assumed the information on this sheet is complete as of the calendar “Date” displayed in cell H5. This field remains editable at will.
Step 4 – Establish The Motorcycle Customer’s Identity
Another piece of preliminary information that should be produced before continuing with a formal request for specific products and work is the identity of the Motorcycle Maintenance Customer. Use cells A11, A12, and A13 to document the “Name,” “Street Address,” “City, State,” and “Country” in the Motorcycle Customer’s address. Conclude the address by dispensing the official postal code assigned to cell A14 (labeled “Zip Code”). Document the “E-Mail” and “Phone” number of Motorcycle Mechanic or Maintenance Shop in cell A15 then in A16.
Step 5 – Distribute The Dates Of Service To The Motorcycle
Proceed to the area of column G titled “Order.” The first field in this section is populated with a label (“Expected Start Date”) that requires a record of the first workday’s date entered.
Produce the first anticipated day of work in cell G11
Cell G12 continues this section with its request for the “Expected End Date” of the motorcycle maintenance job.
Step 6 – Account For The Motorcycle Products Required For The Maintenance Job
The first column in the “Material” table located near the center of the spreadsheet has been reserved for a manifest of the billable motorcycle products. You may list the product name or number row by row in column A (named “Description”). In addition to identifying the motorcycle part or supply needed in the first column, the second column requires you continue across the row with a record of how many units or parts will be necessary. This “Quantity” must be presented in column F of this table.
The third requirement of this table must be satisfied with the “Unit Price” of the listed motorcycle part or supply. This will prompt the generation of a few values in column H. Notice this “Amount” column will inform the Motorcycle Customer how much will be owed for the “Material” required.
Step 7 – Establish The Expected Motorcycle Service Time
The next table also expects a few definitions regarding the work being done. Here, in the “Service” table, a record of how much billable time will be spent on the motorcycle will have to be produced. Use column A (titled with the word “Description) as a listing area for the dates, project names, service specials, or required tasks that can be used to define the time that requires the Customer’s payment. Column F in this table (“Service”) is titled “Hours.” As indicated, this column requests that the total number of “Hours” the Motorcycle Mechanic or Maintenance Shop will spend working on the motorcycle is reported.
The third column holds the words “Hourly Rate” as its title. Here the “Amount” to be paid for the shifts (or other “Description”) should be listed for review. Deliver a production of the dollar amount that one hour of work will cost the Client to column G. In turn, column H will populate several more cells to its content.
Step 8 – Document The Motorcycle Maintenance Sales And Service Tax
Notice that most of the entries necessary to distribute a cost report to the Motorcycle Maintenance Customer have been delivered however cell H36, labeled “Tax,” remains vacant. This content will depend solely upon the jurisdiction whose sales and service “Tax” regulations govern the Motorcycle Mechanic’s or Maintenance Shop’s transactions. If a sales or service “Tax” is expected with the “Subtotal,” then furnish a presentation of the determined figure in cell A36. If no sales or service taxes are applied to the motorcycle maintenance (or repair) payment, then a zero may be recorded here.
Step 9 – Name The Preparer(s) Behind The Concerned Motorcycle Maintenance Request
Once, you have entered the sales or service taxes due, the “Total” will incorporate the new figure accordingly in cell H37. Review this figure for its accuracy, then deliver your full name after the words “Work Order Completed By” in cell A37 to the left. If other Parties delivered entries to this document, their names must also be listed here. The “Individual’s Name” text is only a place holder and should be removed before providing this information.
Step 10 – Additional Maintenance Instructions Or Motorcycle Repair Comments May Be Included
If the Motorcycle Mechanic or the Customer wish to document, present, or include additional material this may be handled in cell A38 at your discretion. This is a free form field that allows for any relevant information, promotions, or incentives (i.e. coupons) to be included with this maintenance request.
Step 11 – Finalize This Request
This request for maintenance or repairs on the Customer’s motorcycle, made by the Customer, must be approved by the Customer. He or she is provided a blank line (labeled “Client Signature”) in cell A42 below the statement confirming his or her authorization for the work above to commence.
